Purchasing a puppy is a meaningful commitment. We care very much about our puppies, and we want to learn as much as we can about you and your family so that we can ensure that we're matching our puppies with families that are prepared to provide them the love and care they deserve. So tell us a bit about you. Completing this form does NOT commit you to purchasing a dog! And please answer honestly; your answers here don't necessarily disqualify you from purchasing a dog from you. This is just our way of understanding what we might need to discuss with you when we contact you for a telephone interview!

Financial Commitment: Our puppies are $2,950. A non-refundable deposit of $500 is required, which will be applied to the purchase price of your puppy. A final payment of $2,450 is due at the time of puppy pickup. Do you understand and agree to these terms if you decide to purchase a dog?
